Providence Within Romeo and Juliet

Shakespeare writes of a world marred by the petulant idiocy of the Montagues and the Capulets, requiring God to intervene in order for the quarrel to end. Romeo and Juliet are fated to be â€Å"A pair of star-cross’d lovers†¦Ã¢â‚¬  and are the living sacrifice necessary to end the feud. Providence utilizes the pressures of society and time to create whirling events established as fate within the play in order to impose a gentle ending for the lovers. Shakespeare establishes a forcefulness of fate and a great will of God in the life and death of Romeo and Juliet as a means to bring peace to the corrupted society of Verona. The public and private lives of Romeo and Juliet are greatly influenced by God’s unequivocal desire to end the disorder and chaos within Verona. Providence utilizes the pressures formed within Shakespeare’s society to aid in the death of Romeo and Juliet. The feud between the Montagues and the Capulets creates an undisputed loathing between the two families and yet, Romeo and Juliet manage to look deeper within identity and see beyond a family name. Juliet transcends beyond Romeo’s surname and knows that her love for Romeo lies within Romeo’s personality. â€Å"What’s in a name? That which we call a rose/ By any other word would smell as sweet. † (2. 2. 43-44) Though the lovers express a maturity in being able to come together privately and look beyond identity, the idea of Romeo and Juliet coming together is inconsistent to society. As a result, the lovers form their lives together in secrecy which results in Providence easily manipulating the course of the lovers as their love is unknown and disregarded within society. The secrecy of Romeo and Juliet’s love, and later marriage, causes Juliet to hurry into swift action which is later revealed. Friar Laurence, characterized by his wisdom and philosophy, is swayed in judgment as he begrudgingly weds Romeo and Juliet. The Friar agrees to oversee the marriage as it is his idea that the two families will be united through a sacrament between the couple and God. â€Å"For this alliance may so happy prove/ To turn your households’ rancour to pure love. † (2. 3. 91-92) Friar Laurence recognizes the need to end the feud as the quarrel between the two families will otherwise completely disunify society through violence and death. However, Friar is aware that the rash and impulsive decision to marry will cause the couple â€Å"[to] stumble as [they] run fast. † (2. 3. 94)- as Shakespeare is referring to the lovers, he evokes the death of the couple. Shakespeare establishes fairness within the law whilst also establishing Providence’s ability to manipulate fate through Romeo’s banishment. Romeo acts out of foolishness as a result of vengeance and fate. Romeo, after killing Tybalt in an attempt to avenge Mercutio’s death, is sentenced, by the law- giver Prince, to banishment as an acknowledgement of Romeo killing the murderer, Tybalt. Shakespeare evokes Providence’s desire to establish unity within Verona as the law is given fairly through Prince and additionally the tragedy of the lover’s demise is continued as a means to end the feud and restore order. Poverty, within Romeo and Juliet, aids in destroying the lives of the lovers. Romeo, stricken with grief at hearing of Juliet’s death, wishes to commit suicide and approaches a poor apothecary whom unwillingly agrees. â€Å"My poverty, but not my will, consents. † (5. 1. 5) Shakespeare establishes that the society he writes of creates a contrast of wealthy and poor resulting in the apothecary providing Romeo with an illegal poison and gaining money the apothecary believes may make him wealthy. The poison the apothecary provides Romeo with causes the physical death of Romeo and aids in the work of Providence as the demise of one lover ultimately kills the other. Romeo and Juliet’s deaths and the events that lead to their demise ar e a constant work of Providence. Shakespeare utilizes speed as evidence of the force of fate acting upon Romeo and Juliet as well as, an aid in the destruction of the lovers. Shakespeare builds a fast- paced world around Romeo and Juliet which results in swift and hazardous decision making. The final scene of the play, which details the death of the lovers, occurs quickly and hastily as Romeo swiftly slays Paris, drinks the poison, and dies within the tomb having Juliet, moments later, immediately commit suicide at seeing her love lying deceased upon the ground. Shakespeare establishes the reliance of time in regards to assisting Providence in the death of the lovers. Events happen at such a rapid pace, it results in an inability for characters to adhere to the Friar’s wisdom of properly contemplating situations and an inability to ponder a solution other than that of suicide which would attempt to counter Providence. The deaths of Romeo and Juliet are necessary sacrifices within Providence’s plan to restore unity, peace, and ease within Verona. The citizens of Verona are greatly influenced by a feud that Shakespeare leaves unexplained and unreasoned. Servants from either household are willing to fight for their masters, though not for a particular reason. â€Å"The quarrel is between our masters and us their men. † (1. 1. 18) Shakespeare emphasizes the need for a great sacrifice as Verona’s citizens are easily swayed into violence without proper reason, suggesting the inability for the law within Verona to overcome the violence resulting from the feud. The feud within Verona has resulted in a loss of human dignity and a disregard for life. Tybalt, upon recognizing Romeo’s voice at the Capulet’s banquet, is quick to assume Romeo has come to harm the Capulet’s in some way and swiftly admits â€Å"To strike him dead I hold it not a sin. † (1. 5. 58) The suicidal death of Romeo and Juliet reestablishes a regard for human life as the families witness the result of their children having lost a regard for their own lives. Providence required the death of the two lovers as their death effected both the Capulets and the Montagues in such a personal way as they realized the consequences of the feud the families had established. Romeo and Juliet are fated by Shakespeare to love and die as a means to end their parents’ feud. Providence is evident within the lovers’ lives and deaths as events occur out of fate. The disordered society of Verona pressures the lives of several characters to unknowingly aid in the destruction of the lovers. Time is evoked as being destructive as the rapid movement of time aids in the lovers’ dying at possibly avoidable moments. The forcefulness of fate and the will of God as a result of the lovers’ parents’ quarrel resulted in the death of Romeo and Juliet.

Manager’s Function, Skills and Role Essay

This page investigates the skills, roles and functions of Management. For any organisation to achieve the goals it has established and be successful it needs managers to correctly implement and understand the functions, skills and roles involved in the managerial process. How these are applied will vary depending on what level of management a particular manager is involved in (high, middle, low) and the organisation. Regardless of these two factors all management decisions focus on the efficient and effective use of resources for the benefit of the organisation, in the direction of its desired goals and/or objectives. The Functions of Management The four functions of management are planning, organising, leading and controlling (Davidson et al, 2009. p.13). Planning involves reviewing the current situation and generating a plan that will allow the organisation to meet its established goals and objectives (Selley, 2009). This could entail generating a plan to increase profit and detail how this will be achieved (focus on capturing a larger market share or perhaps moving into a new market). Correct planning ensures there is a degree of focus, while also providing a structured timeline that relevant stakeholders can adhere too. The second stage is organising, this is where management prepares for the task ahead by delegating resources and responsibilities, as efficiently and effectively as possible (Pakhare, 2011). During this stage management would consider the different departments and divisions within its organisation and provide authority and tasks as necessary. An organisation that wants to increase its profit might use the organising stage to outline the roles of marketing (investigate and promote new market share) and separate these from the accounting department (assess the viability by calculating projected sales and expenses) while also ensuring they all have the necessary resources to complete the work. Davidson et al. defines leading as the process of getting members to work together for a common interest (2009. p.14). Leading requires a manager to have a positive influence on people while also inspiring them to complete their jobs (making this vital in low-middle management), this in turn improves their job performance through a positive work environment (Expert Manage, 2011). It is important to establish this positive environment to ensure that deadlines set in the initial planning stage can be met and resources are not being wasted. The final stage in the function of management is controlling, this stage is important in the establishment of performance standards and ensuring these standards are adhered too while also taking corrective actions against deviation. If for example deadlines are not being met the manager should investigate, if they this was due to incorrect original estimates relating to the workload required, the manager could increase staffing for the particular project. Management has been described as a social process involving responsibility for economical and effective planning & regulation of operation of an enterprise in the fulfillment of given purposes. It is a dynamic process consisting of various elements and activities. These activities are different from operative functions like marketing, finance, purchase etc. Rather these activities are common to each and every manger irrespective of his level or status.Different experts have classified functions of management. According toGeorge & Jerry, â€Å"There are four fundamental functions of management i.e. planning, organizing, actuating and controlling†. According to Henry Fayol, â€Å"To manage is to forecast and plan, to organize, to command, & to control†. Whereas Luther Gullick has given a keyword ’POSDCORB’ where P stands for Planning, O for Organizing, S for Staffing, D for Directing, Co for Co-ordination, R for reporting & B for Budgeting. But the most widely accepted are functions of management given by KOONTZ and O’DONNEL i.e.Planning, Organizing, Staffing, Directing and Controlling.| For theoretical purposes, it may be convenient to separate the function of management but practically these functions are overlapping in nature i.e. they are highly inseparable. Each function blends into the other & each affects the performance of others. 1. Planning It is the basic function of management. It deals with chalking out a future course of action & deciding in advance the most appropriate course of actions for achievement of pre-determined goals. According to KOONTZ, â€Å"Planning is deciding in advance – what to do, when to do & how to do. It bridges the gap from where we are & where we want to be†. A plan is a future course of actions. It is an exercise in problem solving & decision making. Planning is determination of courses of action to achieve desired goals. Thus, planning is a systematic thinking about ways & means for accomplishment of pre-determined goals. Planning is necessary to ensure proper utilization of human & non-human resources. It is all pervasive, it is an intellectual activity and it also helps in avoiding confusion, uncertainties, risks, wastages etc. 2. Organizing It is the process of bringing together physical, financial and human resources and developing productive relationship amongst them for achievement of organizational goals. According to Henry Fayol, â€Å"To organize a business is to provide it with everything useful or its functioning i.e. raw material, tools, capital and personnel’s†. To organize a business involves determining & providing human and non-human resources to the organizational structure. Organizing as a process involves: * Identification of activities. * Classification of grouping of activities. * Assignment of duties. * Delegation of authority and creation of responsibility. * Coordinating authority and responsibility relationships. 3. Staffing It is the function of manning the organization structure and keeping it manned. Staffing has assumed greater importance in the recent years due to advancement of technology, increase in size of business, complexity of human behavior etc. The main purpose o staffing is to put right man on right job i.e. square pegs in square holes and round pegs in round holes. According to Kootz & O’Donell, â€Å"Managerial function of staffing involves manning the organization structure through proper and effective selection, appraisal & development of personnel to fill the roles designed un the structure†. Staffing involves: * Manpower Planning (estimating man power in terms of searching, choose the person and giving the right place). * Recruitment, selection & placement. * Training & development. * Remuneration. * Performance appraisal. * Promotions & transfer. 4. Directing It is that part of managerial function which actuates the organizational methods to work efficiently for achievement of organizational purposes. It is considered life-spark of the enterprise which sets it in motion the action of people because planning, organizing and staffing are the mere preparations for doing the work. Direction is that inert-personnel aspect of management which deals directly with influencing, guiding, supervising, motivating sub-ordinate for the achievement of organizational goals. Direction has following elements: * Supervision * Motivation * Leadership * Communication Supervision- implies overseeing the work of subordinates by their superiors. It is the act of watching & directing work & workers. Motivation- means inspiring, stimulating or encouraging the sub-ordinates with zeal to work. Positive, negative, monetary, non-monetary incentives may be used for this purpose. Leadership- may be defined as a process by which manager guides and influences the work of subordinates in desired direction. Communications- is the process of passing information, experience, opinion etc from one person to another. It is a bridge of understanding. 5. Controlling It implies measurement of accomplishment against the standards and correction of deviation if any to ensure achievement of organizational goals. The purpose of controlling is to ensure that everything occurs in conformities with the standards. An efficient system of control helps to predict deviations before they actually occur. According to Theo Haimann, â€Å"Controlling is the process of checking whether or not proper progress is being made towards the objectives and goals and acting if necessary, to correct any deviation†. According to Koontz & O’Donell â€Å"Controlling is the measurement & correction of performance activities of subordinates in order to make sure that the enterprise objectives and plans desired to obtain them as being accomplished†. Therefore controlling has following steps: * Establishment of standard performance. * Measurement of actual performance. * Comparison of actual performance with the standards and finding out deviation if any. * Corrective action. Management Skills While a solid grasp of management functions is important management should also have a sound skill base, a report by Hay’s Group suggests this is a weak area for most individuals in management and is one of the biggest threats to business success (Financial Advisor, 2007). These skills include but are not limited to technical, interpersonal and conceptual skills. â€Å"Technical skills are those necessary to accomplish or understand the specific work being done in an organisation,† (Davidson et al, 2009. p.19). These technical skills are used mostly at the lower levels of management and are obtained through experience and training. An example of technical skills could be introducing and teaching a new accountant the accounting system used by the firm. Interpersonal skills focus on the ability to work with, motivate and communicate with other people (Hahn, 2007). This skill is very important for managers to create a strong relationship between both individuals and groups within the organisation (Dale, 2008, p.121) this will ensure that there is trust and respect between these parties, allowing tasks to be completed effectively and efficiently. The final core skill of management is conceptual thinking, this is the ability to consider a situation both abstractly and logically in order to come to the correct decision based on internal and external environments (Griffin, 2011. p.179). Globalisation has made this skill extremely important for managers (especially those in high-level positions), as they must be able to break-down and analyse information to make immense decisions that potentially have a worldwide impact. Management Roles Through numerous managerial roles the skills discussed above are put into practice. Mitzberg’s observations and research suggest that a manger’s role can be split into ten roles and three categories; interpersonal (management through people), informational (management through information) and decisional (management through action) (Daft and Marcic, 2010. p.17). Interpersonal roles naturally form an important part of being a manager and relate to activities involving other people. Some interpersonal tasks could involve being a figurehead and speaking at important organisation ceremonies, motivating employees through leadership or serving as a liaison between different departments (Davidson et al, 2009. p.18). Informational roles are focused on the processing of information, this could include seeking out information while also analysing and monitoring for relevant changes that apply to the organisation, communicating information to your co-workers or being a spokesperson on behalf of the organisation (MindTools, 2011). Informational roles require quick and thorough processing of information and resilience to information overload. The final category is decisional which requires decisions to be made using the information provided. This could require developing innovate ideas, serving as a mediator to resolve conflict, allocating resources within the organisation and negotiating on the organisations behalf. And yet, there is such a thing as th e criminalization – the creation of criminals – among the young. Contemporary society has developed a set of practices, targeted at the young of particular minority groups, which tend to skew their development towards increasingly aberrant behavior. Here, we are not speaking of those deviant social elements which are out to corrupt the innocent. We speak of those stewards of social order, the guardians and inculcators of virtue – the police officers, the school teachers, the parents themselves, and similar persons endowed with authority and regarded with high deference within the contemporary social order. There are several mechanisms by which criminalization of the young takes place. For instance, the zero tolerance policy, also known as the â€Å"school-to-prison pipeline† ‘refers to the policies and practices that push our nation’s schoolchildren, especially our most at-risk children, out of classrooms and into the juvenile and criminal ju stice systems’ (H). It is evidenced by the perceptible rise in school based arrests, which ‘most directly exemplifies the criminalization of school children.’ (H) Oftentimes, zero tolerance becomes a tool for racial prejudice because ‘students of color are far more likely than their white peers to be suspended, expelled, or arrested for the same kind of conduct.’ (H) The school is also the unwitting agent of another criminalization mechanism known as the cumulative disadvantage. Together with the zero tolerance policy, cumulative disadvantage takes place when children, suspended or expelled from school for a reason often not of their own fault, are ‘left unsupervised and without constructive activities; they also can easily fall behind in their coursework, leading to a greater likelihood of disengagement and drop-outs’ (H). There are numerous statistics that support these assertions, but compelling as they are, the numbers are not as per suasive as the accounts of the very individuals caught in this self-perpetuating system of prejudice and criminalization. The other half of the bread is eaten. The unleavened bread is a symbol of humility before god. The Jews show the humility as a way of thanking God for delivering them from the slavery. He says nothing could equal my delight at seeing Clerval; He feels same when he is in the presence of his friend, and so his rejection of the monster when he held out a hand for security, makes him a hypocrite. This is confirmed when Frankenstein grasps his hand. Whereas, he turned away from the monster rather than holding his hand like he does with Clerval. The remainder of the chapter tells us about Victors illness and recovery, which is due to his mental state after the monsters creation. In this chapter, many key themes of the book are highlighted. We see examples of friendship and rejection; we see the ideas of parent/child relationships and the way victor rejects his responsibilities, purely based on his looks, and therefore, we are also presented with ideas about prejudice. We also have views about whether the creation of the monster was morally acceptable or not. In chapter four, we see how Frankenstein rejects the responsibilities that he has over the creature which he has brought to life. We can see that he has no intention of looking after the creature when he says I sincerely hope, that all these employments are now at an end, and Im at length free. This tells us tat he was feeling restricted, almost imprisoned because of his creation, but he is now free, therefore he has no intentions of caring for his creation like a father should. The use of the word employments tells us he thinks of him as being a chore (like a job is) and so instead of loving him, he dreads and hates him in the same way you do a job. We can also see how victor has rejected him in the words he uses to describe him. He does not give him a name, which is a duty performed by all parents, instead he refers to him as the dreaded spectre and hideous guest. The use of the word spectre, refers to the way Frankenstein obviously wishes the creature would die, so he does not have any responsibilities. The idea of naming a human is to give it identity, so it can be recognised, however, Frankensteins refusal to do show, tells us he does not care about him and thinks that by ignoring him, and not giving him an identity, he will go away. In this respect, he is neglecting his parental responsibilities. We then see him running away from it when it tries to reach out to him for security and as a symbol of love. He currently has no warm feelings towards the monster, who we can only presume has run away, when he was rejected by his father. Throughout the rest of the novel, we see Frankenstein continually denying his presence, and only thinks of him at times when horrible events have taken place. He presumes him to be the murderer of his brother William, purely based on his deceptions which he gained from his appearances. In order to confirm this, he seeks him out to ask his story. They then meet in the cold mountains in chapter 2 of the second volume. This is two years after his creation, in which the monster has learnt to talk and has also learnt many other skills any other human would have had. This is the only time Frankenstein shows any sentimentality towards his creation, and so he decides to listen to his story in order to discover what he has been doing. He says I felt what the duties of a creator towards his creation were i ought to render him happy. Frankenstein has now acknowledged his status as a father and decides to let the monster tell his story to make him happy. He also seeks to find out who really did kill his brother, and if his suspicions can be proved. However, earlier in the chapter, he describes himself as the miserable origin and author The word miserable tells us he deems himself to be a monster, and the use of the same word also to describe the monster in chapter four also shows how he can draw a parallel between them both. The reasons for Frankensteins hatred of the monster, comes from his opinion that you have to be beautiful to be a decent person. He tried to make the monster like this, but when he realises the monster was ugly, he immediately presumes him to be evil and as something to fear. This discriminatory view is the reason why Frankenstein rejects the monster. He describes the work to create him as the beauty of the dream. As the reality of the dream is not so beautiful, Victor might see the monster as someone who has ruined his dreams and therefore, he hates him for that. However, his prejudicial views are what appear to make him hate the monster, although this might be a contributing factor. Prejudice is another key idea Shelley presents to us in the novel. We can see Frankenstein discriminating against the monster when the creature first comes to life.

He loved very much children, in their presence he felt good, happy because in his soul he was still a child. Case Study: Pre Eclampsia | Essay This essay is based on a case study and will discuss the underlying pathophysiology, management and psychosocial aspects a patient faces when diagnosed with pre-eclampsia. Information was obtained through the patient and from their medical notes. Section A: Case History JB is a 38-year-old, gravida 4, para 0, Caucasian female who presented in hospital at 36 weeks plus 5 days gestation with sudden development of oedema in the face and upper and lower extremities along with a severe headache. On examination her blood pressure (BP) was elevated at 171/107 mmHg. Her pulse 81 beats per minute (bpm), respiratory rate 15 breaths per minutes, and temperature was 36.4  °C. Her urine sample showed ++ 2 proteinuria. There was evidence of oedema in her face and upper and lower extremities and her lower deep tendon reflexes were brisk but without any clonus. JB denied any visual disturbances and epigastric pain. On palpitation of the abdomen, the symphysio-fundal height was 38cm. The fetal lie was longitudinal and the back appeared to be on the right. The presentation appeared to be cephalic and the head was 3/5 engaged. Electronic fetal monitoring showed fetal heart rate at 135 bpm, with reassuring variability. There was no deceleration and acceleration was normal. It was also noted that contractions were absent. Her antenatal care had been shared between the hospital and her general practitioner and was uneventful until she was admitted to hospital on this occasion. On briefly reviewing JBs past medical history she has seasonal allergies as well as long standing asthma in which she uses salbutamol inhaler to relieve her symptoms. JB also has a history of depression. In her family history her father suffers from hypertension as well as cirrhosis and her mother has a remarkable medical history of extensive medical conditions such as hypertension, angina, and transient ischemic accidents. Both her maternal grandparents had a history of type 2 diabetes. With regards to her social history, JB works as a customer assistant and lives with her partner. JB has never smoked, and has not taken alcohol since finding out she was pregnant. Her past obstetric history is remarkable for recurrent miscarriages. She had two miscarriages at 6-8 weeks and one ectopic pregnancy a year and a half ago which miscarried naturally at 10 weeks. The initial investigations showed; a normal full blood count, liver enzymes and creatinine. However urate (0.37 mmol/l) and the protein: creatinine ratio (44 mg/mmol) levels were elevated. JB is demonstrating key cardinal symptoms of pre-eclampsia including hypertension, proteinuria, oedema, and increased reflexes.1 A diagnosis of severe pre-eclamptic toxaemia was made, JB was admitted and treatment was commenced with 10mg nifedipine. With JB being admitted onto the ward, there was continuous monitoring of BP and fetal monitoring using cardiotocography (CTG). The next morning JBs BP stabilised to 128/74 mm Hg and she reported feeling better. With the BP stable and a reassuring CTG a decision to induce delivery was made and she was given Prostaglandin E2 (PGE2) over three days. However there was poor response and the cervix remained obstinately unchanged and so it was decided the baby would need to be delivered via caesarean section. Section B: Pathophysiology Pre-eclampsia is part of a range of conditions known as the hypertensive disorders of pregnancy.2 It is defined as a multisystem disorder characterised by the new onset of raised BP (≠¥140/90 mm Hg) and proteinuria (at least 1 + on dipstick or ≠¥0.3 g/24 hours) after 20 weeks of gestation.3-5 Although the triggering event initiating the syndrome is unknown, a two stage model of pre-eclampsia has been proposed (figure 1).1,6-9 The primary asymptomatic stage, characterised by inadequate remodelling of the placental vasculature during the first trimester results in reduced placental perfusion leading to placental ischemia and release of placental products into the maternal circulation.1,6,7 Consequently this initiates the second symptomatic stage, the maternal syndrome in which endothelial dysfunction precedes the clinical manifestations of the disease including characteristic hypertension, proteinuria, and glomerular endotheliosis.1,6,7 There is also risk for developing the H ELLP syndrome (haemolysis, elevated liver enzymes and low platelets), progression to eclampsia, and end-organ damage.1,6,7 Stage 1 In normal pregnancy, following implantation, the surface trophoblast cells of the adhering blastocyst differentiate into an inner cellular layer, the cytotrophoblast, and an outer syncytiotrophoblast.10,11 The undifferentiated cytotrophoblasts found in the inner layer can develop into hormonally active villous syncytiotrophoblasts, extravillous anchoring trophoblastic cell columns, and invasive intermediate trophoblasts.10 The extravillous trophoblastic cells proliferate from the tips of anchoring chorionic villi to form the cytotrophoblast shell which line the uterine cavity.10,11 Cytotrophoblasts continue to migrate through the uterine endometrium until they reach the spiral arteries, by which time they have differentiated into an endothelial-like cell type.10,11 Endovascular trophoblast cells begin to remodel the spiral arteries by replacing the endothelium and smooth muscle cells resulting in the destruction of medial elastic, muscular and neural tissue.10-12 These physiological changes result in an increased vessel diameter leading to the creation of a low-resistance arteriolar system with the absence of maternal vasomotor control, and therefore allowing a notable increase in blood supply to the developing fetus.7,10 In pre-eclampsia this physiological dilatation does not occur adequately thus resulting in placental hypoperfusion and ischemia.1,8,10 The ischemic placenta may lead to the production of cytokines and growth factors as well as simulate placental apoptosis or necrosis, resulting in release of humoral or particulate materials into maternal systemic circulation that promote generalised maternal vascular endothelium dysfunction, culminating in the clinical manifestations of pre-eclampsia.1,8,10 The invasion of trophoblast cells is regulated by factors expressed by the decidual barrier.7,10 These factors include cell adhesion molecules, extracellular matrix, proteinases, growth factors and cytokines.7,10 Malfunctions in any these factors may lead to poor trophoblast invasion and subsequently pre-eclampsia.7,10 Immunological factors play a main factor in pre-eclampsia.7,10,11 Abnormal placentation may be the result of maternal immune rejection of paternal antigens expressed by the fetus. Normally HLA-G, a class 1B MHC antigen, expressed by the extravillous trophoblasts protects from natural killer cell lysis.11-13 Women who develop pre-eclampsia do not appear to express this HLA-G and therefore are not protected.7,10 Stage 2 The clinical manifestations of pre-eclampsia can be linked to the pathophysiological changes that occur including vasoconstriction, activation of coagulation cascade and reduced plasma volume.14 Development of hypertension is a key feature of pre-eclampsia.1,8,14 During normal pregnancy, although through maternal physiological adaptations there is a 30-50 % increase in cardiac output, the decrease in peripheral vascular resistance results in decreased arterial BP.14 However, those who develop pre-eclampsia experience widespread vasoconstriction, increased peripheral vascular resistance, and decreased cardiac output.1,13 Evidence shows there to be an exaggerated sensitivity of the vasculature of women with pre-eclampsia to all vasopressors hormones, best known is the increased responsiveness to angiotensin II.1,13 This increase in vascular reactivity is thought to be due to an alteration in the balance of prostaglandins as a result to the damage to vascular endothelial damage.1 JBs BP when she presented was 171/107 mm Hg. This is a considerably elevated and can be defined as severe pre-eclampsia (≠¥160 / ≠¥110 mm Hg).2-4 Although fluid retention and oedema occurs in patients with pre-eclampsia they are also a feature of normal pregnancy.1 Plasma volume increases by approximately 50% in uncomplicated gestations and normal gravidas sometimes experience oedema.1 However in pre-eclampsia plasma volume is decreased by 15-20% and in these cases women experience rapid weight gain and generalised oedema as a result of an abnormal shift of extracellular fluid (ECF) from the vascular to the extravascular compartment hence maintaining a low plasma volume and an increased interstitial fluid volume.1,14,17 With excessive accumulation of interstitial ECF, the presence of peripheral oedema particularly in the face and hands is seen. JB haemoglobulin was 130g/l, reflecting the haemoconcentration seen in pre-eclampsia as a consequence of the reduced intravascular volume.1 Women with pre-eclampsia also have noticeably reduced renal plasma flow (RPF) and glomerular filtration rate (GFR).1 The decline in RPF is attributed to vasoconstriction, whereas the fall in GFR is related both to the decline in RPF as well as to the morphological changes in the kidney.1 These characteristic pathological changes of pre-eclampsia are known as glomerular endotheliosis and comprise mainly of diffuse glomerular endothelial cell swelling, enough to obstruct the capillary lumen.1,7,8,13,15 The morphologic lesion is accompanied by functional changes in renal hemodynamics which correlates best with the magnitude of hyperuricemia and proteinuria.1,5 Early to middle pregnancy serum uric acid usually falls well below 0.24 mmol/l, in patients with pre-eclampsia levels often rise >0.27 mmol/l as seen in the case of JB (0.37 mmol/l).14 A number of studies have correlated the rise in serum uric acid with the severity of pre-eclampsia and with the extent of glomerular injury.5,8 JBs PCR levels was also measured to estimate the extent of proteinuria.
